$180 for the initial session; $155 per hour after that.


Insurance
I am a Preferred Provider (PPO), in-network for for Anthem Blue Cross/Blue Shield, Meritain, CHA, United HealthCare, UMR, Select Health Network, Aetna. (If you live in Michigan and have Michigan BCBS, please note that I will not show up on your list of preferred providers, but I will show up on the list of providers out of state. You will be covered because you are seeking treatment out of state.) I am NOT able to take Medicaide, nor Healthy Indiana Plan (HIP) or MDwise. Please check your cove rage by calling your insurance company and asking:

  • Do I have outpatient mental health insurance benefits? Note, health insurance companies do NOT cover couples or marital counseling as they do not consider it a medical necessity.
  • Do they cover tele-health counseling?
  • What is my deductible and has it been met? Do I have a per visit copay?
  • Is pre-certification required?

Payment: Cash, check or Visa/Mastercard/Discover Card at time of appointment.

Cancellation Policy
As I have reserved my time for you, I ask that you contact me by 7:00 am if you will not be able to make your appointment so that I may offer that time to other clients. There is a $50 late cancellation fee if you do not call to cancel by 7 am on the day of your schedule appointment. This is your bill, it is not something that can be billed to insurance.

Good Faith Estimate Notice

Notice to clients and prospective clients:

Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.  If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
